44th Annual NCAPA Virtual Summer Conference

This 44th Annual NCAPA Summer Conference covers a variety of topics which are geared toward primary care providers, including Assessment of the Returning Traveler, Mini EKG Boot Camp, "WWYD?" Critical Thinking: Case Studies in Medicine, and Culturually and Critically Conscious Communication. Sessions are selected and reviewed by our panel of PAs to provide a general review of and update on topics that are the most useful to providers. This conference includes 2 hours of controlled substances CME required by the North Carolina Medical Board for PAs licensed in the state of North Carolina and 3.25 Pharmacology Credits (Pain Management in the Older Adult - 1 credit, Opioids: The Good, The Bad, and The Deadly - 1 credit, Street Drugs - 1 credit, Early Pregnancy Loss - 0.25 credit).

Rheumatic conditions are not uncommon and can be managed with a systematic approach to holistically treat persons with these diagnoses. While numerous advances have been made in the treatment of rheumatic diseases in recent decades, these treatments require knowledge of their risks, benefits, and expectations. The pharmacologic treatment of diabetic kidney disease (DKD) has typically involved treatment of associated risk factors such as blood glucose, blood pressure, and the use of medications that act on the renin-angiotensin-aldosterone system. Recently, a variety of glucose-lowering medications have been investigated to assess their potential for improved renal outcomes in patients with type 2 diabetes mellitus and with/without established DKD. Several of these medications have been shown to improve renal outcomes in patients with type 2 diabetes mellitus. This presentation seeks to provide insight into the results of these clinical trials and how the selected medications can be used to individualize therapy as recommended in updated guidelines issued by the American Diabetes Association and the American Association of Clinical Endocrinologists/ American College of Endocrinology.

This includes understanding our own implicit bias and how it can affect our assessment and treatment of patients. Migraine headaches affect both men and women in the United States. Migraine is a highly prevalent disorder that affects 11% of the U.S. population. It is the source of significant suffering for individuals and their families. In fact, approximately 17% of women and 6% of men had at least one migraine attack in the previous year. Treatment options have changed with new classes of medications entering the market. Providers need to be up to date on the most current recommendations for the acute and preventive treatment options for migraine patients. This presentation will use variable case studies to illustrate the subtleties that inform providers about the correct diagnosis and most effective treatment options for migraine patients.

A recent student found more than 37 million Americans were prescribed antidepressants in 2019. The COVID-19 epidemic is expected to increase the number of prescriptions. A significant number of these prescriptions were for serotonin reuptake inhibitors. While rare, serotonin syndrome can be a potentially life-threatening condition. A sentinel injury is an injury occurring prior to child sustaining documented child physical abuse. Dr. Sheets documented in her 2013 paper that 27.5% of children who presented with definite child physical abuse had had a sentinel injury prior to that and that 8% of the children with intermediate concern for child physical abuse had had a sentinel injury prior. Sheets et al, Sentinel Injuries in Infants Evaluated for Child Physical Abuse, Pediatrics 131(4) 701-707 March 2013. Almost 42% of the children who had the sentinel injury prior to their larger injury had a medical provider who was aware of the injury.  We are missing these and leaving children in potentially harmful/deadly environments. This lecture will cover what are the most common sentinel injuries and what to do when they are found. Also resources available in NC and SC to help providers when they have concerns. It will also review the new NC law regarding mandated reporting.

Because of these critical uses of death certificates and their data, it is essential that death certificates be filled out completely, accurately, and promptly. This presentation will provide principles and common pitfalls in death certification with the goal of improving death certification accuracy as well as what deaths should be reported to the medical examiner system for death certification by a medical examiner.
